Check voltages are correct, check that the input signals are not too high and try a pass program where you only read input and write to output, no other processing.

What is the level of the signal you are putting into the chip?
Does a pass program work and pass signal?

You need to post details of your hardware, example are using a circuit board you have designed yourself or are you using the Spin development board. If you are using your own design, have you fitted a crystal as no crystal would make the LED clip light all the time. Similar if you are using an external EEPROM and it has no programs flashed to it this again would light the LED.
You need to provide details of what you have and what you are doing otherwise any of us trying to help would be useless guessing.

If the LED is on all the time that can mean the oscillator has not started or the EEprom is empty or not working.
